Facilities Request Form

If you would like to reserve a space at TDF for your event, please fill out the form below as completely as possible. Be sure to choose the space(s) you need from the floor plan map and list them in the space provided.

If there are no conflicts with another event or questions about your event, you reservation will be accepted and you will receive an email confirmation. If there are any fees associated with the event, you will be contacted to arrange a time to come to the church to fill out a facility agreement and to discuss the fees. If there is a conflict with another event, you will receive a call to discuss alternative arrangements.
